Regional Care Management Shared Services

Funding from the American Rescue Plan Act was used by the NENCPPH to pilot sharing of services across the region for two care management programs: high-risk pregnancy (CMHRP) and at-risk children ages 0-5 (CMARC).  Recruitment and training for two float care managers, one for each program occurred prior to these floats being deployed.
